Please have this completed form and if your pet will be staying with us,
a copy of your pet's vaccination records available for us at or before the date of service.

**IMPORTANT!**
Please be sure to provide a copy of your pet's UP TO DATE
vaccination information if they will be staying overnight with PPPS

All pets staying at PPPS are required to be current on all their vaccinations, including:

For DOGS
1. DHPP (4-in-1 vaccine of Distemper, Hepatitis,
Parainfluenza, & Parvovirus)
2. Rabies
3. Bordetella
4. Fecal Exam

For CATS
1. Rabies
2. Feline Distemper,
3. Feline Leukemia,
4. Fecal Exam

Any animal found to be not up to date on these vaccines or found to be infested with fleas or ticks will be treated &/or vaccinated at the owner’s expense.
